重建鎖定鋼板治療胸鎖關節脫位的近期療效_《中國修復重建外科雜志》

目的探討重建鎖定鋼板治療胸鎖關節脫位的近期療效。方法回顧分析2008年2月－2012年2 月，采用重建鎖定鋼板治療的11例胸鎖關節脫位患者臨床資料。男7例，女4例；年齡30～55歲，平均 44歲。致傷原因：交通事故傷8例，砸傷3例。受傷至手術時間為2 h～11 d，中位時間6 d。均為胸鎖關節前脫位。根據Grade分型標準：Ⅱ型2例，Ⅲ 型9例。結果術后患者切口均Ⅰ期愈合，無神經、血管損傷等并發癥發生。術后2 d X線片檢查示，胸鎖關節脫位均達滿意復位與穩定固定。患者均獲隨訪，隨訪時間9～24個月，平均16個月。術后9個月，根據Rockwood等的評分方法評定為10～15分，平均13.2分；獲優9例，良2例，優良率達100%。均無內固定失效及再脫位發生。術后9～15個月取出內固定物，肩關節穩定性及活動良好。結論對胸鎖關節脫位采用重建鎖定鋼板內固定具有穩定性好、脫位復位滿意、允許早期功能鍛煉、最大程度恢復肩關節功能的優點，近期療效滿意。
