目的　系統評價總前列腺特異性抗原（t-PSA）為4～10 ng/mL時血清游離前列腺特異性抗原/總前列腺特異性抗原（f/tPSA）用于前列腺癌診斷的臨床價值。
方法　計算機檢索中國生物醫學文獻數據庫（1999～2009）、中文科技期刊全文數據庫（1999～2009）、中國期刊全文數據庫（1999～2009）和萬方數據庫（1999～2009），并手工檢索相關期刊，全面收集f/tPSA診斷前列腺癌的中文文獻，按QUADAS條目對納入研究進行質量評價。采用MetaDisc 1.4軟件檢驗各研究間的異質性，并按f/tPSA臨界值0.1、0.15、0.2、0.25及0.3分組，進行Meta分析。
結果　最終納入18個研究。異質性檢驗提示無閾值效應，但存在其他因素導致的異質性，按隨機效應模型做Meta分析。結果顯示f/tPSA 臨界值為0.15組的敏感度與特異度均較好，SROC曲線下面積及Q指數分別為0.883 5和0.814 0，明顯高于其他組。
結論　f/tPSA比值是前列腺癌診斷灰區（t-PSA為4～10 ng/mL）的較好診斷指標，以0.15作為臨界值較為適合。
