腔鏡在下肢靜脈性潰瘍治療中的應用_《中國普外基礎與臨床雜志》

目的探討腔鏡在下肢慢性靜脈功能不全(CVI)并發靜脈性潰瘍中的臨床治療經驗與療效。方法回顧性分析2004年5月至2011年4月期間我院應用腔鏡治療78例(88條患肢)下肢CVI并發靜脈性潰瘍患者的臨床資料，患者均行大隱靜脈高位結扎+腔內激光治療(EVLT)+腔鏡深筋膜下交通靜脈離斷術(SEPS)。結果所有患者手術順利。SEPS手術時間15～30 min，平均20 min；術中出血量1～5 ml，平均2 ml；術后住院時間2～8 d，平均5 d。術后肢體酸脹感和曲張淺靜脈消失，色素沉著區縮小。術后筋膜下血腫3例，皮下氣腫2例，小腿脛前區及足靴區麻木感3例。所有患者4～6周潰瘍愈合，隨訪0.5～5年，平均3.5年，僅1例復發，是由于足靴區交通靜脈殘留。結論SEPS是治療CVI并發靜脈性潰瘍的首選方法，具有創傷小、出血少、手術時間短、恢復快、并發癥少、療效顯著等特點。
