改良法重建拇長伸肌腱功能的療效觀察_《中國修復重建外科雜志》

目的通過與傳統示指固有伸肌腱轉移術比較，探討改良法重建拇長伸肌腱功能的療效。方法2009年1月－2011年12月，采用改良法重建拇長伸肌腱功能治療11例拇長伸肌腱自發性斷裂患者（改良組）。手術在傳統術式基礎上，增加對拇長伸肌腱近側斷端與示指固有伸肌腱加固縫合，以及示指固有伸肌腱遠端環繞拇短伸肌腱反折縫合。術后采用SEEM（specific EI-EPL evaluation method）評價方法評定手術療效，并與同期采用傳統示指固有伸肌腱轉移術治療的18例患者（傳統組）進行比較。兩組患者性別、年齡、病程及致傷原因等一般資料比較，差異均無統計學意義（P gt; 0.05），具有可比性。結果術后兩組患者切口均Ⅰ期愈合。傳統組5例失訪，其余患者均獲隨訪，隨訪時間8個月～2年6個月，平均1年6個月。末次隨訪時根據SEEM評價方法，改良組拇指抬高丟失及屈曲丟失均顯著小于傳統組，差異有統計學意義（P lt; 0.05）；示指單獨背伸丟失與傳統組相似（P gt; 0.05）。改良組獲優9例，良2例，優良率100%；傳統組獲優5例，良6例，可2例，優良率為84.6%；兩組優良率比較，差異有統計學意義（χ2=0.03，P=0.03）。結論與傳統示指固有伸肌腱轉移術相比，改良法重建拇長伸肌腱功能手術操作簡便，增加了轉移肌腱力量，近期療效滿意，但遠期療效需進一步隨訪。
