低位/超低位直腸癌結肛吻合術中運用單吻合器及雙吻合器技術的療效比較△_《中國普外基礎與臨床雜志》

目的　比較在低位/超低位直腸癌結肛吻合術中運用單吻合器技術（SST）和雙吻合器技術（DST）進行吻合重建的效果。
方法　回顧性分析2009年1月至2010年12月期間四川大學華西醫院胃腸外科中心結直腸外科專業組收治的351例低位/超低位直腸癌患者的臨床資料，比較采用SST （n=49）和DST （n=302）進行結肛吻合患者的術中和術后情況。
結果　與DST組比較，SST組患者的腫瘤下緣距齒狀線距離較短（P＜0.05），遠端切緣長度較短〔（1.83±0.59） cm比（2.07±0.56） cm，P＜0.05〕，手術時間較長〔（112.86±39.29） min比（100.10±36.75） min，P＜0.05〕，住院費用較低〔（24 350.48±7 812.73）元比（29 455.32±7 869.33）元，P＜0.05〕。而2組患者的術中出血量，首次下床活動時間，首次排氣、排便時間，拔除胃管、尿管及引流管時間，術后住院時間、總住院時間及術后并發癥發生率比較差異均無統計學意義（P＞0.05）。術后所有患者的肛門控便功能均恢復良好。術后全部獲訪，隨訪時間6～24個月，平均16個月。隨訪期間，局部復發1例（SST組）；遠處轉移3例（均為DST組）；死亡15例（4.27％），其中DST組13例（4.30％），SST組2例（4.08％）。
結論　低位/超低位直腸癌結肛吻合術中SST的遠端切緣長度較DST短，適用于腫瘤位置較低的患者，并且其住院費用也較DST低。
