目的　研究Roux-en-Y胃轉流術（RYGB）對2型糖尿病（T2DM）大鼠的治療作用，并探討內臟脂肪組織產生的絲氨酸蛋白酶抑制劑（vaspin）在RYGB治療T2DM機制中的可能作用。
方法　取造模成功的T2DM大鼠20只和周齡、性別相匹配的正常SD大鼠20只，用隨機數字表法將其隨機分為T2DM-RYGB組、T2DM-假手術組及RYGB組、假手術組，每組10只。分別于術前及術后第4和8周檢測各組大鼠的空腹血糖（FPG）、血清胰島素（INS）、血清vaspin水平及胰島素抵抗指數（HOMA-IR），并分析血清vaspin水平與FPG、INS及HOMA-IR的相關性。
結果　手術前，T2DM-RYGB組與T2DM-假手術組比較以及RYGB組與假手術組比較，FPG水平、INS水平、vaspin水平及HOMA-IR差異均無統計學意義（P＞0.05）；而T2DM-RYGB組及T2DM-假手術組的FPG水平、INS水平、vaspin水平及HOMA-IR均分別明顯高于RYGB組（P＜0.05）及假手術組（P＜0.05）。術后第4周，T2DM-RYGB組FPG水平、INS水平、vaspin水平及HOMA-IR較術前下降，除FPG水平（P＜0.05）外，其余指標與術前比較差異均無統計學意義（P＞0.05）；術后第8周，FPG水平、INS水平、vaspin水平及HOMA-IR進一步下降，與術前比較差異均有統計學意義（P＜0.05）。T2DM-假手術組、RYGB組及假手術組組內術前及術后第4周、第8周FPG水平、INS水平、vaspin水平及HOMA-IR比較，差異均無統計學意義（P＞0.05）。手術前及手術后第4周、第8周T2DM-RYGB組與T2DM-假手術組血清vaspin水平與其對應血清INS水平、HOMA-IR均呈正相關（P＜0.05）。
結論　RYGB對T2DM大鼠具有一定的治療作用，RYGB后vaspin表達水平降低，胰島素敏感性改善，這可能是RYGB治療T2DM的機制之一。
