目的　探討胰十二指腸切除術中采用胰胃吻合重建胰腺殘端的手術指征及療效。
方法　回顧性分析上海同濟醫院2005年8月至2008年2月期間施行的胰十二指腸切除并行胰胃吻合的38例患者的臨床資料。
結果　本組患者均順利完成手術，平均手術時間(352.1±78.3) min, 術中平均輸血量(911.3±601.4) ml；術后住院時間平均(26.2±12.1) d; 術后發生并發癥8例(21.1%)，其中胰瘺1例(2.6%)，胃排空障礙2例(5.3%)，切口感染2例(5.3%)，腹腔積液1例(2.6%)，肺部感染2例(5.3%)，均經保守治療后痊愈，無住院手術死亡病例。
結論　胰胃吻合是一種安全、可靠及有效的胰腺殘端重建方式，根據患者的具體情況合理地選擇胰胃吻合可以最大程度地減少胰十二指腸切除術后胰瘺的發生。
