高脂血癥對孕期大鼠急性胰腺炎的影響_《中國普外基礎與臨床雜志》

目的研究高脂血癥對孕期大鼠急性胰腺炎發生的影響，并探討其可能機理。方法 72只妊娠大鼠按抽簽法分為高脂飲食組和均衡飲食組，分別喂養16 d后，用L-精氨酸鹽酸鹽溶液（250 mg/100 g）進行腹腔內注射，1 h后再次腹腔內注射（200 mg/100 g）。分別于末次注射后0、12、18、24、30和48 h各取6只大鼠檢測血清甘油三酯、淀粉酶和脂肪酶，計算胰腺濕/干重比值，觀察胰腺組織的病理變化并評分，免疫組化SP法檢測胰腺組織TNF-α表達。結果各時相高脂飲食組甘油三酯均較均衡飲食組明顯升高（P＜0.05）；高脂飲食組淀粉酶及脂肪酶的峰值分別在24 h及18 h出現，均衡飲食組延至30及24 h時出現且明顯較高脂飲食組水平低（P＜0.05）；在L-精氨酸鹽酸鹽溶液末次注射后12、18和24 h，高脂飲食組的胰腺濕/干重比值較均衡飲食組明顯升高（P＜0.05）,TNF-α表達也更高（P＜0.05）；在0、12、18及24 h，高脂飲食組胰腺水腫出血、炎癥浸潤更重，病理評分更高（P＜0.05）。結論高脂血癥能使L-精氨酸誘導的孕期急性胰腺炎提前出現，后果更嚴重，可能是胰腺炎發生、發展的一個危險因素，其機理可能是高脂血癥時甘油三酯大量釋放游離脂肪酸，上調TNF-α的表達。
