【摘要】目的探討重癥急性胰腺炎（SAP）中IL-10及IL-18的表達及其與SAP肺損傷的關系。
方法將48只SD大鼠按隨機數字表法均分為對照組和SAP組，動態定量測定肺濕重系數、腹水量、血清淀粉酶、血清IL-10及IL-18的變化和半定量RT-PCR檢測肺組織中IL-10 mRNA及IL-18 mRNA的表達，并在光鏡下觀察胰腺及肺組織病理學改變。
結果各時相SAP組肺濕重系數、腹水量、血清淀粉酶、血清IL-10及IL-18水平和肺組織中IL-10 mRNA及IL-18 mRNA的表達與對照組相比均明顯增高（P＜0.05,P＜0.01）。SAP組大鼠血清中IL-18水平和肺組織IL-18 mRNA表達水平均與肺濕重系數（r=0.68，P＜0.01； r=0.72，P＜0.01）及肺損傷病理評分（r=0.74,P＜0.01; r=0.79,P＜0.01）呈正相關，而血清中IL-10水平和肺組織IL-10 mRNA表達水平均與肺濕重系數（r=-0.62, P＜0.01; r=-0.69, P＜0.01)及肺損傷病理評分（r=-0.66,P＜0.01; r=-0.60,P＜0.01）呈負相關。
結論 IL-18參與了SAP并發肺損傷的病理過程，而IL-10對肺損傷有保護作用。
